A black plaque on a dark wooden base. There is an emblem on the plaque with an eagle and a maple leaf in the center and Canadian Warplane Heritage Preserve and Maintain written along the outside of the emblem. The plaque says, "Presented to the RCAF WD Association of Hamilton To express our sincere gratitude for your support of the Canadian Warplane Heritage Museum 'Partners in Perserving Canadian History'".
